ONLINE REGISTRATION

To complete online registration:

  • Click Here to complete online registration.  You will be able to register multiple teams for each tournament, provide names for each team, and indicate the level of play for each team.
  • Submit payment online via credit card or mail a check payable to Achieve Lacrosse.
  • Complete online registration by emailing team rosters to Carla Farkes at Carla.Farkes@gmail.com.
  • All participants must complete the Online Player Waiver and Tournament Release prior to the start of the tournament.

INCLEMENT WEATHER POLICY

Inclement weather is out of the control of the tournament directors and officials.  In the event of thunder and/or lightning, all games will be suspended for 20 minutes.  Games will resume, in real time, 20 minutes after the last occurrence of thunder and/or lightening. Canceled games may not be rescheduled.  If weather disrupts scheduled games, the Tournament Director will make every effort and has the sole authority to adjust the schedule as needed, including, shortening games, adjusting the schedule, etc.  In the event of extreme weather, it is up to the tournament directors and officials as to whether the conditions are deemed safe to play on.

TOURNAMENT CANCELLATION POLICY DUE TO INCLEMENT WEATHER

Xtreme Lax is an outdoor tournament and therefore is subject to the threat of inclement weather.  In the event of weather that causes complete or significant cancellation of tournament play, Achieve Lacrosse reserves the right, in its sole discretion, to offer either partial refunds to participating teams or credits towards future participation in an Achieve Lacrosse event.  There shall be no refunds for individual games or playoff games lost to weather delays or unplayable field conditions.  Clubs will be notified if any weather related refund/credit shall be provided within 30 days following the event.   Achieve Lacrosse shall not be responsible for refunding individual player fees paid to any given Club for participation in an Achieve Lacrosse event.

INSURANCE POLICY & DISCLAIMER

Achieve Lacrosse is insured by US Lacrosse for general liability and accident insurance through RPS Bollinger (on the condition that the event operator, all players and head coaches are active US Lacrosse Members.)  Through Bollinger Insurance, US Lacrosse provides exclusive on-the-field coverage for all US Lacrosse members participating in Achieve Lacrosse tournaments.

All players attending Achieve Lacrosse Tournaments must complete an online player registration, waiver and tournament release prior to participation.  In order to complete the online form, all participants MUST provide a current US Lacrosse membership number and expiration date.

Captains, coaches or directors must provide a team roster which will include team name, player name, jersey number, address, and emergency contact phone number. Rosters must be submitted by email in an excel format to Carla.Farkes@gmail.com prior to the start of the tournament.   A hard copy of the completed tournament roster should also be turned in to the tournament director on the day of the event so that it is on hand for the athletic trainer(s) in the event of an emergency.

Achieve Lacrosse will review the player registrations and waivers submitted from each team, and let captains, coaches or directors know the status or their team’s compliance with player registration. It is the responsibility of captains, coaches or directors to make sure that all players have completed the Achieve Lacrosse tournament participation player registration, waiver and US Lacrosse membership verification.
